This is not as simple as it might seem. It is a greeting the same as saying hello or hi and a native English speaking person is in all honesty NOT interested in how you are. You ought to reply with a simple 'fine thanks',you? / I'm great, are you ok ? or that type of thing.? Which means you are returning the greeting.
