How many 3-0 comebacks are there in MLB?
In the history of these leagues, teams that were down 3–0 in a series have come back to win the series five times; four times in the NHL and once in MLB. The most recent example is the Los Angeles Kings of the NHL in the 2014 Stanley Cup playoffs.
The unfortunate answer is zero times. While no teams have come back to win after being down 3-0, there are three teams that have rallied to tie up a series 3-3 and went on to lose Game 7.
It's a matter of probability. Suppose two teams are equally matched such that each team has a 50% chance of winning any particular game. Coming back from a 3–0 deficit requires winning 4 consecutive games, with a probability of only 6.3% (=. 5^4).
In the history of these leagues, teams that were down 3–1 in a series have come back to win the series 58 times; 14 times in MLB, 13 times in the NBA, and 31 times in the NHL. The most recent instance was accomplished by the New York Rangers of the NHL in the 2022 Stanley Cup Playoffs.
In the NBA's 75-year history, 146 teams have faced a 3-0 series deficit in a seven-game postseason series. Of those teams, 14 have won at least two games while three have won three straight games to force a decisive Game 7. None have come back to win the series.

Are 7-Game World Series More Common Than Expected?
In Major League Baseball, there has been one successful comeback from a 3-0 deficit, and that occurred when the Boston Red Sox stormed back to defeat the New York Yankees 4-3 in the 2004 ALCS.
Rarest of all is the unassisted triple play by an outfielder, performed only once in professional baseball history, by Walter Carlisle. It is possible for a team to score on a triple play, but that is also phenomenally rare.

This was one of only four World Series to go to eight games, and the only best-of-seven Series to do so. By rule, a batter can not cause a pitcher to balk. A balk is the result of an illegal action by the pitcher while any base is occupied. If the batter leaves the batter's box with both feet during the pitch and the pitcher delivers the pitch, two strikes are called on the batter.
