What personality traits should I use, if I want to create the most evil CK3 charcter?
Do you guys have any suggestions regarding, other traits that I should use?

sad*stic and Callous are a given, the third depends on what kind of evil you want to play. You can play an arbitrary tyrant, a gluttonous cannibal, a deceitful/vengeful murderer, a greedy pig imprisoning, torturing and banishing courtiers/vassals for their money/land, a lustful seducer ruining other people's marriages... there are many options.

Reactions:
sad*stic is a must.
Ambitious characters love war and usually don't mind betraying people.
Paranoid sees problems even when there isn't any.
Zealous can be pretty cruel (even to their own faith sometimes).
For other traits:
Lunatic and Possessed can do pretty bad stuff by accident (like killing friends).
Irritable isn't too nice either.
Avoid the Imbecile trait: they can't start seduce schemes. Which reminds me, you could make a Lustful Seducer Beautiful Rakish character to spread diseases as much as possible...

Callous and sad*stic are mutually exclusive, you can't have both. That said, I'd say sad*stic, Arrogant, and Deceitful would be a pretty nasty combo.

sad*stic as people said is a must.
Wrathful is also one that is quite dangerous. a sad*st who is quick to anger is not fun to be around.
make Aerys Targaryen Paranoid,Wrathful,Craven & sad*stic ? cant think of anything more terrifying. someone who gets mad when threatened, feels threatened by everything and enjoy causing suffering.

I will keep this in mind, thanks.

I'd probably go with sad*stic, wrathful, and vengeful. Callous technically is mutually exclusive with sad*stic in normal gameplay, but can be added by ruler designer.

I recall berserker (for Norse pagans) can give nasty surprise like murdering half of your court in rage. On the other hand, borderline (or straight) insane characters could be excused as not truly evil (because ther're mentally ill). Cannibal is a must, though.
Callous technically is mutually exclusive with sad*stic [...], but can be added by ruler designer.
No it can't? I did test it just in case:
And even if it could, what would be the point? sad*stic is much worse than Callous, they want to make others suffer.
